Before your initial ‘New Patient Visit’

• [x] Complete the Intake Packet

• [ ] Complete ‘Primary Care Referral’ before Dec. 2024 (Instructions).

  Referral Form and Letter to your PCP

• [ ] Register ****for our secure patient portal.

• [ ] **Schedule** your ADHD Assessment Results Visit at about 1 week from your initial ‘new patient visit’.

New Patient Visit ———(1st appointment)

• [ ] Complete the QbCheck Test using the emailed link and voucher code sent after your initial visit.
• [ ] Complete the ADHD Profile using the emailed link sent after your initial visit and share it with your provider (how to share).
